    If you've been thinking about starting therapy, the first question is usually a practical one: "Will my HMO pay for this?" The honest answer is: sometimes. It depends on your plan, your provider, and the type of professional you want to see.

    Here's a clear, no-fluff guide to what HMOs in the Philippines typically cover for mental health, and what to do if yours doesn't.

    The short answer

    Most HMOs in the Philippines cover psychiatry as a medical specialty, meaning consultations with a psychiatrist (a medical doctor) are usually reimbursable, similar to seeing a cardiologist or dermatologist.

    Coverage for psychologist sessions and counseling is inconsistent and heavily plan-dependent. Some corporate plans include it. Many do not. The safest move is to verify with your HMO or HR before you book.

    What's typically covered, and what isn't

    Psychiatric consultations are the most consistently covered. Because psychiatrists are medical doctors, HMOs treat the visit like any other specialist consultation. Medication prescriptions are usually included; the medications themselves are often not.

    Psychologist therapy sessions, meaning talk therapy with a Registered Psychologist or Psychometrician, are a different story. Many standard HMO plans do not include them. Higher-tier or corporate plans sometimes do, often with a session cap (e.g. 4 to 8 sessions per year).

    Counseling with a Registered Guidance Counselor is rarely covered by HMOs, even though counselors are PRC-licensed mental health professionals. If covered, it's usually under an Employee Assistance Program (EAP) arranged by your employer, not your HMO directly.

    Telehealth and online therapy coverage varies. Some HMOs reimburse online psychiatric consults if the provider is accredited. Independent online therapy platforms typically aren't reimbursable, but it's worth asking, since policies have been shifting since 2020.

    One of the first major HMOs in the Philippines to formalize mental health benefits through a Specialized Care Program for Mental Health. Psychiatrist consultations are typically covered for eligible plans, often with a session cap.

    MediCard

    Mental health benefits vary widely by plan. Corporate group plans are more likely to include psychiatric consultations and, in some cases, a limited number of psychologist sessions. Individual plans are typically more limited.

    Intellicare

    Corporate-focused. Mental health coverage is usually tied to the employer's specific plan design. If you're an Intellicare member, your HR is the fastest source of truth on what's included.

    PhilCare

    Has the most dedicated mental health coverage among major Philippine HMOs, including a Mind Care Plus add-on that explicitly covers psychiatric and psychological services. If mental health access matters to you when choosing a plan, this is one to look at.

    PhilHealth is separate from your HMO

    PhilHealth is the national health insurance program. It's not the same as your private HMO, and your benefits stack. PhilHealth's Outpatient Mental Health Package, rolled out in recent years, covers up to ₱9,000 annually for general mental health care and up to ₱16,000 annually for specialized care, claimable through accredited facilities.

    In practice, most people still pay out of pocket because the package is only honored at PhilHealth-accredited mental health facilities, the documentation requirements are heavy, and many private psychiatrists and psychologists aren't enrolled in the program. It's worth asking your provider if they file PhilHealth claims, but don't count on it.

    What to do if your HMO doesn't cover it

    If your plan doesn't include therapy, or only covers a few sessions, you still have practical options.

    • Ask HR about an EAP. Some companies offer a separate Employee Assistance Program with confidential counseling sessions, distinct from the HMO.
    • Look at sliding-scale clinics. Some training institutions and NGOs offer therapy at reduced rates with supervised practitioners.
